All registry information is intended to be made widely available on the Web. Please fill out the form completely and candidly. DO NOT include any sensitive personal information you do not want viewed by the general public. We do ask that you provide a current email address that will serve as your primary contact vehicle. To select more than one item in a list, hold the CTRL key while making your selections. When you have centered information on the form, you may PREVIEW your submission. When you are done, select SUBMIT. If your submission is successful, you will be taken to a Thank You page.

What we're looking for on these sections:
- Leadership Interest: Select what type(s) of positions for which you would you like your record to be considered. If you don't see a selection, please enter your response in "Other".
- Professional Experience: Select areas in which you have high sector, industry and functional (e.g., marketing, operations, etc.) knowledge and competence. If you don't see a selection, please enter your response in "Other".
- Community Service: Select service positions in which you have a demonstrated track record of results. If you don't see a selection, please enter your response in "Other".
- Personal Leadership Statement: Your "commercial" - a brief statement that summarizes: a) your leadership expertise, b) your professional, industry, and volunteer experience and competence, and c) how you create value (results) within the organizations with which you are associated and the community at large.
- Political Affiliation: If you want to be considered for a government commission and perhaps other government roles, you will need to make a selection in this field besides "Other". Ethnicity may also be required for diversity reasons.
